Online Webform
Click the button below and complete the various fields, then click submit. These web forms are submitted to the parish records department to be processed into the database. You will receive a confirmation auto-reply when your web form is submitted. Please allow a minimum of one week for processing.
In Person
Office: Please call 518-836-5319 for hours to stop by the parish office located in the Rectory at 1241 State St.
The building is next to the church, set back from the street. The door faces the church.
